The Department for Community-Based Services (DCBS) is committed to building a strong and supportive system of care for Kentucky citizens and communities.

The survey covers topics such as:

  • Your experience with DCBS staff
  • The effectiveness of our programs and services
  • How well we are meeting the needs of Kentucky communities.

This survey is completely voluntary, and your responses will be anonymous. Information gathered from the survey will be used solely to assess our performance and make improvements to our programs and services.
