2013 Kentucky Coordinated School Health Summit -

TEAM REGISTRATION -
This Summit is designed to bring teams together from across Kentucky to focus on a coordinated school health approach to school district wellness policy planning and program review. The Local Health Department Administration will register their local team. Each registered teams participants will receive updated information about the Summit including an agenda and information on reserving a hotel room.
INFORMATION NEEDED FOR REGISTRATION - General information needed for registering your team includes the name, title, organization and email address for each member in attendance. Please register the minimum of 4 team members with a maximum of 6 team members for your local team. Please recall that the pre-work required for the Summit is completion of the WellSAT school district policy assessment tool. The WellSAT total Comprehensiveness Score and WellSAT Total Strength Score are both needed to register the team.
Goal: School districts, with stakeholder input, will develop a plan to improve their school district wellness policies and a process for assessing implementation of their school district wellness policies.
As a result of this training, participants will be able to:
1. Apply knowledge gained through pre-work and summit sessions to overall school district planning and policies
2. Expand the school district wellness policy and practices to address Coordinated School Health (CSH) components
3. Develop an action plan that outlines next steps for expanding the school district wellness policy to include CSH components
